Heart rate averaged 180 beats per minute and respiratory rate averaged 32 breaths per minute. Pink granulation tissue was present and epithelialization is occurring around the perimeter of wound. Silver alginate dressing with CarraVet Acemannan wound cream was applied and covered with tegaderm. Cast padding followed by vet-wrap in figure 8 pattern applied. Secured with tape. Wound on the carpus on the left wing was cleaned with chlorohex solution and silver sulfadiazine cream was applied. Recovery unremarkable. Returned to enclosure. Plan is to continue meloxicam. Next bandage change on Tuesday.

A chewing louse was found on 2017-246's head. Treated with fipronil spray.

The culture collected from the wound over the metacarpal-phalangeal joint during surgery had heavy growth of Klebsiella pneumoniae with suceptibility to numerous antibiotics. Trimethoprim/sulfa (TMS) was chosen to treat this infection due to its large volume of distribution and decreased risk of drug resistance. As 17-246 has already been receiving this antibiotic, we will continue the course of TMS.

Procedure -

Surgery time - 73 minutes

Surgery performed on 10/24/17:
17-246 was transported to AUSATH for surgical repair of the right
metacarpal-phalangeal joint. The patient was placed under general
anesthesia for the procedure. After a sterile surgical scrub,
the wound was debrided of necrotic and non-viable tissue as well
as the articular cartilage of both epiphyses to promote healing
and closure of the joint. Three 0.045 K-wire pins were placed
perpendicular (ventral to dorsal) to the long axis of the major
metacarpal just proximal to the joint, and another three were
placed perpendicular (ventral to dorsal) to the long axis of
the phalanx just distal to the joint. The wire ends were bent
and subsequently covered in a bridging casting material (VetLite)
to cover the wire ends and stabilize the joint. Post-operative
radiographs were taken to confirm apposition of the bones; appropriate
alignment was achieved. In recovery, a figure-eight bandage was
placed after application of SSD cream to the joint. The patient
recovered well and was subsequently transported back to SRC for
post-operative care.

SSD cream was applied and a Tegaderm covering was placed. A splint was made out of two tongue depressors and was placed on the dorsal surface of the fracture. A figure eight wrap was then placed.

Changed the bandage this morning. The wound is moist and soft tissue distal to the wound is still swollen but is warm. Used a saline soaked applicator to clean the wound, minimally, and replace SSD into the wound. The area around the wound was clean and dry, so I place a small tegaderm over the wound to keep it moist. Padded the wound, replaced the tongue depressor and placed a figure 8 bandage over the wing.

If the bandage is in good shape tomorrow will delay bandage change to Monday. BAEA was very good for the bandage change.

It has no food left in the cage.

Isoxiprine was obtained from the pharmacy and is added to the treatments today, 1ml/orally/once daily.

Isofluorane was used to place 17-246 under a moderate plane of anesthesia for radiographs. Lateral and craniocaudal views were taken of the right thoracic limb. A luxation of the right major metacarpal and phalangeal joint was evident along with a fracture of the right minor metacarpal.

ADDITIONAL DIAGNOSTICS:
-An orthopedic consultation was performed while 17-246 was under anesthesia. The ventral tendon of the joint is ruptured but the dorsal tendon appears to be intact. A mild amount of debris was still present within the wound. There was a moderate amount of edema.

PROCEDURES:
- The splint was reapplied using SSD cream 4x4 dry gauze, covered with two-inch vet wrap, a splint using tongue depressors was placed on the dorsal aspect and a figure eight bandage applied.

PLAN:
-17-246 will undergo orthopedic surgery next Tuesday. The goal for the joint is arthrodesis. An external fixator will be placed on the dorsal aspect of the luxation to stabilize the joint as well as allow access for wound management. The cartilage of the joint will be debrided with a scalpel blade to stimulate formation of scar tissue and eventually bone creating arthrodesis of that joint. The wound will also be cultured at this time to determine the appropriate antibiotic.
-17-246 will continue to have daily bandage changes until the wound has a good bed of granulation tissue.
-17-246 will continue to receive daily antibiotics (TMS 48mg/kg) and anti-inflammatory therapy (Meloxicam 1mg/kg)
